WHO QUALIFIES FOR HOSPICE CARE?
Patients whose physician has given a prognosis of approximately six months, or less, and who are in agreement with the philosophy of Hospice care are eligible for hospice services. As long as the patient still meets Hospice criteria, Hospice care can continue after six months. |
WHO PAYS FOR HOSPICE CARE?
Faith Hospice is a certified Medicare / Medicaid Hospice Provider. If eligible for Hospice care, all Hospice services related to the terminal illness are covered by Medicare or Medicaid. Most private or commercial insurance plans offer Hospice benefits.

WHO WORKS FOR HOSPICE?
Hospice care is provided by a team of highly qualified professionals. Faith Hospice, Inc. is owned and operated by a Certified Hospice and Palliative Care Nurse and was created to provide the highest quality of care for patients to truly help them and their families at their time of greatest need. We work as a team with the patient, their doctor and family members to develop a Plan of Care that respects the wishes of the patient and family. The members of Faith Hospice Team include:

WHAT DOES HOSPICE PROVIDE:
Hospice services are provided in the comfort of the patient’s home or in a nursing home.
Hospice services include:
- All medications related to the terminal diagnosis
- Durable medical equipment related to the terminal diagnosis (hospital beds, wheelchairs, walkers, etc.)
- Supplemental oxygen and other equipment and supplies as defined by the plan of care
- Nursing care by professional hospice nurses with knowledge of state-of-the art pain and symptom management
- Home Health Aides to provide help with daily activities such as bathing, feeding, light housekeeping, etc.
- Social Workers to assist with emotional support and connecting patients and families with community resources
- Chaplains to assist patient and family with spiritual needs or to facilitate communication with established relationships
- Trained hospice volunteers to visit or sit with patients or allow caregivers time for their personal needs
- A hospice nurse and doctor on call 24 hours a day, 7 days a week, to give patients and families support and care when needed
